A hospital full of cartoon nurses sounds like an easy sell for a sex comedy. Biing! makes it a demanding job. Keeping patients happy involves nudity, but it also involves admission, supplies, treatment and bills, all while the clinic edges toward bankruptcy. The bodies are exaggerated, the medical jokes are vicious, and the management is much less forgiving than the premise suggests.

That mismatch is both its interest and its central problem. This is a substantial simulation, with enough financial pressure to keep a strategy player occupied after the joke wears thin. For someone coming chiefly for sex, the reward is meagre: brief illustrated nudity, limited motion and little room for a relationship. Biing! can make the hospital absorbing. It struggles to make the paperwork sexy.

## Nudity on the payroll

You hire and assign nurses, doctors and other hospital workers, each an occupational caricature. Nurses' body measurements sit alongside their qualifications; doctors bring golf handicaps to the recruitment process. The exaggerated designs are memorable, and the absurd hiring priorities set the tone. These people have jobs and punchlines, however, rather than much individual development. Chemistry rarely gets beyond the hospital's interest in a woman's body.

Striptease is where the sexual joke earns its place in the simulation. A nurse can entertain patients waiting for treatment, and the game links undressing with morale and recovery. Clicking for nudity therefore does double duty: a brief erotic reward and a small management benefit. That is a neat fit for the premise, but the pleasure is limited to illustrated skin and modest animation. The sexual variety is narrow; the strip button becomes as familiar as the other hospital controls.

The joke also depends on a power imbalance. Women are assessed and ordered around by their employer, with their bodies treated as a hospital resource. The surrounding institution is rotten too: incompetent staff, extortionate treatment and hired thugs keep the farce from being merely a parade of boobs. That broader absurdity gives the humour bite, without supplying the nurses with much agency. The shamelessness can amuse; the shortage of individual character makes repeated stripteases harder to care about.

## Keeping the clinic solvent

The business starts with land, rooms, staff, furniture and medical supplies. Then each patient needs admission, somewhere to wait, an examination, suitable treatment and a bill. Getting people through that chain competes with hiring, purchasing and the next arrival. Enough paying customers must reach the end before daily costs swallow the hospital. A new department can sink you if there are too few patients to pay for it.

Reputation makes those financial choices less straightforward. Poor treatment and aggressive bills can damage how patients regard the clinic, while advertising and nicer surroundings compete with wages and rent. Buying better equipment means giving up some of the cash that could keep you afloat. Later departments, inpatient care and ambulance competition broaden the business, adding worthwhile problems to solve. This expansion supplies much more variety than the erotic illustrations do.

The opening is punishing enough to obscure that depth. Demand is uncertain, money is tight, and an early mistake can finish the business before its systems become rewarding. Training and experience eventually qualify staff for independent work, so the game does offer relief from constant commands. Reaching that point takes patience: employees leave for courses, shifts need cover, and the early patient routine requires frequent intervention. Expansion initially gives you more things to check before it gives you breathing room.

## Good cartoons, bad information

The hand-drawn rooms are the game's strongest attraction. Exaggerated staff, improbable equipment and absurd treatment descriptions give this hospital a comic identity of its own. Sex and medical jokes inhabit the same crowded, grubby world, so the nudity fits the setting instead of appearing as a detached prize. Its erotic appeal depends heavily on a taste for those cartoons. Limited movement leaves little scope for the physical expression that could make the women feel more alive.

The same illustrations are poor guides to the business. A nurse or doctor can remain pictured when the assigned employee is absent, and equipment purchases do not reliably make the room a useful inventory display. You have to consult menus and records to find out what is actually happening. An attractive nurse who is effectively painted onto the room is a good illustration of the problem: the art keeps the fantasy visible while concealing a staffing gap.

That confusion becomes more costly as the clinic grows. Personnel, purchasing, finance and patient information live on separate screens, turning a basic decision into a tour of the interface. Sound offers little respite. The music and digitised noises can give the farce character, but repeated cries and office effects become wearing. The presentation knows how to sell a grubby cartoon hospital; it is much less helpful when you need to run one.

## The CD game and its German-only extras

GOG's package centres on the finished 1995 hospital game in its CD edition, available in English and German. Sexual humour and nudity are already included. The bonuses contain a separately listed uncensored disk version in German only, plus the German-only resort sequel Biing! 2. Both are distinct from the CD hospital game reviewed and scored here; the sequel's holiday business does not expand the first game's systems.

Those distinctions matter when buying for the adult content. The CD edition changed some artwork and reduced the original emphasis on nurses' measurements in hospital rankings. The German disk extra preserves a different historical version. It supplies neither an English adult patch nor a promise of expanded modern sex scenes, and its older setup and version choices need care. The included manual is also German, adding another obstacle for English-only newcomers to an already demanding, text-heavy simulation.

The US GOG list price was $5.99 in October 2026: a reasonable price for curiosity about a complete, eccentric old game and its extras. The first game runs through DOSBox, and GOG lists Windows, macOS and Linux. Its preservation changelog records Windows 10 and 11 compatibility checks. That package helps keep Biing! accessible, but the awkward design comes with it. The larger expense is the attention required to reach another brief nude cartoon.

## Verdict

Biing! is worth considering for patient strategy players curious about a grubby, eccentric hospital sim. Its staffing, reputation and cash decisions have substance, and the striptease mechanic makes nudity part of the business. Yet the brutal opening, scattered information and repetitive patient handling demand more attention than the cartoons repay. The $5.99 US GOG list price recorded in October 2026 makes it an affordable historical curiosity. For sex, intimacy or expressive erotic animation, it is a poor choice: the nurses have memorable silhouettes, but little chemistry, and the menus do most of the work.

### Is the English GOG edition uncensored?

The English game is the CD edition, which changed some of the original artwork and mechanics. GOG includes a separate uncensored disk version in German only. That bonus does not provide an uncensored English edition or an English adult patch.

### Is Biing! 2 included in English?

GOG includes Biing! 2: Sonne, Strand und heiße Nächte as a German-only bonus. It is a separate resort sequel; this review and its scores cover the first hospital game.

### Can employees eventually work independently?

Yes. Experience and training courses can qualify staff for independent work, which you enable through personnel controls. Early staffing, supplies and patient handling still need considerable attention, and employees attending courses leave shifts to cover.

### Which desktop platforms does the GOG release support?

GOG lists Windows, macOS and Linux for the DOSBox-powered first game, with English and German installers. Its November 2024 preservation changelog records Windows 10 and 11 checks. Those checks do not guarantee compatibility with every current machine or establish the same support for every bonus game.
